The Wellbeing Team: Health Coaches

BACK TO MAIN INDEX

A health and wellbeing coach works alongside social prescribers providing regular coaching sessions to support patients develop their skills and confidence to reach their own health and wellbeing goals.

Up to six 45-minute sessions (face-to-face, video call or telephone over a period of 3-6 months) are available for patients who feel ‘stuck’ with some aspect of their lives and want to move themselves forward by setting a goal(s) to work towards with support from a Health and Wellbeing Coach. This works particularly well for those living with a long-term health condition but can also help people regain self-belief, confidence and motivation through the encouragement provided by the coach. The coach can have a longer conversation with patients about non-clinical issues than would be possible with a GP. Coaches support patients to make positive changes, shift perspectives and reframe their thoughts and feelings beyond their current situation to live happier, healthier lives.
