Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible.

If you feel you need a home visit, please call the surgery by 11am if possible and give as much information as you can to the Care Navigators. They will put you name on the visit triage list or advise you on an alternative course of action if appropriate.

Our Home Visits are triaged daily by either an Advanced Nurse Practitioner or a GP.  After requesting a visit, you may receive a telephone call to discuss your request further. You may find your problem or concern can be dealt with on the telephone, and if a home visit is indicated you will be visited by a member of our team.

Home Visits

We have several people working in our team, and you may receive a telephone call or a visit from an Advanced Nurse Practitioner, a Community Frailty Practitioner, a District Nurse or a General Practitioner.

Please be mindful that a practitioner can see several people in the surgery in the time it takes to visit someone at home, so please only request a home visit if you are housebound, or too unwell to travel to the surgery. We allocate the home visit on the urgency of your request and unfortunately you cannot request a specific clinician.
