Emma is a Social Prescriber, working with patients to improve their wellbeing by providing support, guidance and information for a range of non-clinical issues and needs. Social prescribing offers a person-centred approach based around what matters to the individual and will be tailored to suit the person’s unique interests, needs and circumstances. Emma is also the practice Carers Lead, providing support for and awareness of the needs of unpaid carers.
