Advanced Nurse Practitioners & Frailty Team

The Nurse Practitioners work side by side with the GPs here. They are able to work independently to diagnose and prescribe for a wide variety of conditions that would have traditionally been provided for by a GP. In some circumstances they can discuss your case further with a GP or arrange an appointment for you.

The Frailty Team are responsible for assessment and co-ordination of care and services to patients registered as housebound and to those in care/residential homes.
