Our Care Co-ordinators help to identify patients who might need support to manage their health conditions.
They also identify patients who need a medical review and arrange appointments for specific clinics. They also work alongside social prescribers and clinical staff to create pathways for easier patient access to healthcare, especially for those with long-term conditions.
